A short note
What the form asks, and what it doesn't
A contact form is not the place for your health. Here is exactly what it takes.
What the form collects
Your name
Your email and phone
How you would like me to reach you
The time of day that suits you
The message you choose to write
What it never asks for
A condition, symptom or reason for the visit
Medicines, test results or scans
Your date of birth or an insurance ID
Photos or documents
Where your health details go
I ask about your health in the room, on paper or on the practice's own forms page, never in the contact form. Please don't put medical details in a message to me.
Anything you tell me stays private. I share it with your clinic or midwife only if you sign a permission, and only what I did in my room.
Ready to write?
Name, a way to reach you and a time that suits are enough.
